Employee Paperwork is completed on the first day of hire. The employee paperwork consists of Federal, State, Licensing, and Nest required forms for individuals working in a child care setting. All forms are required and must be completed in its entirety.

Please make copies of drivers license/state IDsocial security card and resumé for our records.
 

Required Examinations/Screenings

Michigan Child Care Background Check Consent and Disclosure 

Fingerprinting (every 5 years) A request for fingerprints can be initiated here

Physical Exam (annually)

TB Test (every 2 years)

The above exams and screenings are required before a new staff member can start and are required to be updated as noted above.

 

Required Education/Training/Certificates/Licenses/Certifications

Official College/University Transcript or copy of Degree. Transcripts for college, diploma or certificate of High School or GED Please make copy of staff's highest form of education

Child Development Associate (CDA) or Degree Please make copy of staff's CDA certificate or degree

First Aid Certification Please make copy of current first aid certification

Infant/Child CPR Certification Please make copy of current CPR certification

All employees must create a profile on MiRegisty.org to link their training to Nest Child Care and Parent Institute and to record future trainings to their profile.

Please make a copy of recent training certificate and/or transcript showing successful completion of the course. If staff does not have a current or most recent required training or certification it must be completed within the first 30 days of employment.

Employee Training should be documented through miregistry.com so that their training records can be accessed by licensing and Nest.

Employee Files should be organized using the checklists below. Documents should be organized in the file in the order of the checklist. Medical related documents should be kept separate from the personnel file. Both the medical and personnel files should contain a third party access log.
