CACFP FORMS

A teacher takes a hand, opens a mind, and touches a heart.

The CACFP (or food program) is funded by the Michigan Department of Education. It allows licensed child care centers like Nest to get reimbursed for meals, snacks and formula as long as we follow the state's requirements. As a result, we also have to record attendance, costs and other administrative tasks associated with managing the program. Use the forms below to capture the requirements for managing the food program at your location.

ANNUAL DOCUMENTS

All staff are required to complete the CACFP training. This training is completed annually as a team.

 

MDE RESOURCES

Utilize the Michigan Department of Education (MDE) resources available to meet ongoing CACFP training requirements or to administer the program at your location.

REQUIRED ENROLLMENT DOCUMENTS

​The following documentation is required during enrollment for each child/family as applicable to the family's needs during their tenure at Nest.

CACFP BINDER SETUP

​Each location will need to set their site's CACFP Binder up following the format of the CACFP Program Compliance Cover Checklist.

CLAIMING  

The following forms are used for claiming. If the links do not work you can access them online at www.michigan.gov/cacfp. REMINDER: Meal attendance is to be recorded at the time of service and all meals should be served family-style according to HighScope.

Meal Attendance (Daily) Use this form to record daily attendance for meals.

Meal Attendance Summary Record Use this form to tally attendance for meals served in all classrooms.

 

Average Daily Attendance Instructions Use this form for instructions on how to complete the Average Daily Attendance sheet that is completed monthly.

Average Daily Attendance Use this form to calculate the average daily attendance. At the end of each day, count each student who attended. At the end of the month, add the daily attendance totals, then divide the total by the number of days food was served. This is your average daily attendance.

Summary of Costs The Summary of Costs captures the food and non-food expenses. Tally up all the receipts. Separate the food from the non-food costs and place the receipt value in the appropriate column. If you need help determining allowable costs review Allowable and Non-Allowable Costs. All costs must be supported by receipts, invoices and billings.  

Time and Attendance The Time and Attendance is used to capture how much time Nest staff spends on serving meals and snacks as well as completing all the administrative form requirements. Review the Time and Attendance Instructions or a Sample Time and Attendance to further understand how to complete the form. Please note: You can do this once and copy for the rest of the months as time and attendance for managing the food program should be the same month to month.

Center Food Menu A copy of your monthly food menu is required with each months CACFP food claim. If your site is catered include a copy of the menu provided by the food service company. If you are preparing your own food, include a copy of the menu you created. You can refer to CACFP's sample menu for assistance in preparing a menu for your center.

0-5 Months Infant Menu This menu is used to represent what the infants eat every day since they do not eat from the regular menu. Use this form to record what type and how much milk each infant 5 months and younger are consuming. Follow the Infant Menu Record Instructions for more details on how to complete the form. Please note: You can do this once and copy for the rest of the months until the infant is 6 months then you can add them to the 6-11 menu.

6-11 Months Infant Menu This menu is used to represent what infants eat every day since they do not eat from the regular school menu. Use this form to record what type and how much milk each infant 6 - 11 months are consuming as well as any baby food that is served. Follow the Infant Menu Record Instructions for more details on how to complete the form. Please note: You can do this once and copy for the rest of the months until the infant is 12 months or until the infant is eating from our regular food menu.

 

Please double check all math to make sure you are calculating the attendance and receipts correctly. All forms should be completed in its entirety including signatures and dates.

FAMILY STYLE DINING  

Family-style dining in a HighScope classroom involves children and educators sitting together at a child-sized table to share meals, fostering social and emotional development. It encourages conversations, participation, and learning about food and eating habits in a relaxed and engaging atmosphere. The following are training videos to assist with staff understanding how to incorporate family-style dining in the classroom.

CACFP: FAMILY STYLE DINING This video provides CACFP specific training for incorporating family-style dining at the center.

CLASSROOM: FAMILY STYLE DINING This video provides classroom insight as to what family-style dining looks like in the childcare setting and why it's an essential part of the daily routine. We use family-style dining to promote independence and create more learning and development activities.

 

File all Food Program (CACFP) documents in the CACFP Binder along with all receipts and invoices as well as the food menu for the claim month.
