Our enrollment process captures DHS enrollment and eligibility from the start. However, we know that throughout a family's enrollment and tenure at Nest their eligibility and enrollment status with DHS changes and shifts. This section is to help families through the enrollment and eligibility process.
DHS APPLICATION & VERIFICATION
DHS/CDC Application The DHS/CDC application is for families to apply or reapply for child care benefits.
DHS/CDC Provider Verification or DHS-4025 The DHS/CDC provider verification is for families to select/authorize Nest as their child care provider.
DHS LETTERS
The DHS Letters are used to follow up on child care subsidy. The letters are designed to follow the different phases of a family's subsidy status while enrolled. In general, a family eligibility for subsidy may require renewal, verification or they may no longer be eligible and have to re-apply. Use the templates below to assist families with following up on their child care subsidy.
DHS Letter - NO LONGER AUTHORIZED
DHS Letter - INCOMPLETE APPLICATION
DHS Letter - NEW AND RETURNING STUDENTS
DHS Letter - PROVIDER VERIFICATION
DHS Letter - NO LONGER INTERESTED
All forms should be completed in its entirety including signatures and dates. Please make note of deadlines and be sure to follow up with the parent at such deadline. We pay our bills with DHS funding so staying on top of a family's DHS status is critical to the smooth operation of Nest Child Care and Parent Institute.
Also, ALWAYS make a copies for the child's file.
